Thoracolumbar and Lumbar Burst Fractures

چکیده
Approximately 79,000 spinal fractures occur annually in the United States. Roughly three out of every four fractures involve either the thoracic or the lumbar spine1,2. The most common site of injury in the thoracic and lumbar spine is the thoracolumbar junction, the mechanical transition zone between the relatively rigid thoracic spine and the more flexible lumbar region3-5. The thoracolumbar junction receives 35 to 45% of the torsional and shear forces on the spine. This flexibility, combined with the curvature arising from biomechanical adaption to upright posture, makes the thoracolumbar junction more prone to injury than the lumbar spine 6.

Predictable Imaging Signs of Cauda Equina Entrapment in Thoracolumbar and Lumbar Burst Fractures with Greenstick Lamina Fractures
STUDY DESIGN A retrospective study. PURPOSE The aim of present study was to investigate imaging findings suggestive of cauda equina entrapment in thoracolumbar and lumbar burst fractures. OVERVIEW OF LITERATURE Burst fractures with cauda equina entrapment can cause neurologic deterioration during surgery. However, dural tears and cauda equina entrapment are very difficult to diagnose clinic...
متن کاملNeurological deficit and canal compromise in thoracolumbar and lumbar burst fractures.
PURPOSE To assess whether canal compromise determines neurological deficit in thoracolumbar and lumbar burst fractures. METHODS 105 patients aged 17 to 60 (mean, 34) years who had burst fractures in the thoracolumbar (n=82) and lumbar (n=23) regions were included. Fractures were classified according to the Denis classification. The extent of spinal canal compromise was assessed by computed to...
متن کاملPosterior Ligamentous Complex Injuries Are Related to Fracture Severity and Neurological Damage in Patients with Acute Thoracic and Lumbar Burst Fractures
PURPOSE The proposed the thoracolumbar injury classification system (TLICS) for thoracolumbar injury cites the integrity of the posterior ligamentous complex (PLC). However, no report has elucidated the severity of damage in thoracic and lumbar injury with classification schemes by presence of the PLC injury. The purpose of this study was to accurately assess the severity of damage in thoracic ...
متن کاملCommentary: "minimally invasive corpectomy and posterior stabilization for lumbar burst fracture".
Eck and collaborators are to be congratulated for detailing an elegant treatment approach to thoracolumbar burst fractures in their ‘‘Minimally invasive corpectomy and posterior stabilization for lumbar burst fracture’’ case report [1]. Although satisfying, their report leaves us with questions that these authors (and the hundreds of contributors before them) have been unable to answer. Some of...
متن کاملEvidence-Based Medicine of Traumatic Thoracolumbar Burst Fractures: A Systematic Review of Operative Management across 20 Years
Study Design Systematic literature review. Objective The management of traumatic thoracolumbar burst fractures (TLBF) remains challenging, and analyzing the levels of evidence (LOEs) for treatment practices can reform the decision-making process. However, no review has yet evaluated the operative management of traumatic thoracolumbar burst fractures with particular attention placed on LOE from ...
